The 2023–24 Macedonian First League is the 32nd season of the Macedonian First Football League, the highest football league of Macedonia. It began on 6 August 2023 and is scheduled to end on 19 May 2024. Struga are the defending champions, having won their first title in 2022–23.[1][2]
The winners qualify for the Champions League first qualifying round. The second-placed team and 2023–24 Macedonian Football Cup winners qualify for the Conference League first qualifying round. The tenth-placed team qualify for the Macedonian First Football League play-off against the third-placed team from the 2023–24 Macedonian Second Football League. The bottom two teams are relegated to the 2024–25 Macedonian Second Football League.

Results
For matches 1–22, each team plays every other team twice (home and away). For matches 23–33, each team plays every other team for the third time (either at home or away). The league standings after matches 22 are used to determine the games for the last 11 matchdays.
Positions by round
The table lists the positions of teams after each week of matches. In order to preserve chronological evolvements, any postponed matches are not included to the round at which they were originally scheduled, but added to the full round they were played immediately afterwards.

The tenth-placed club will face the third-placed club from the 2023–24 Macedonian Second Football League for the final place in the following season's Macedonian First Football League.
